To thank you for recommending new patients to us, we will credit your account with $65 for each and every patient you refer.*

• The person you refer must attend for dental treatment.

• There is no limit to the number of patients you refer.

• The credit will be made to your account on the day your referred patient pays their account for treatment.

• Your credit must be used within 12 months from the date of entry to your account.

• By advising us in writing, you may transfer your credit to another patient in this practice.

• Your credit is for dental treatment only, not to purchase dental products.
